You are on a continuing contract, hold a Professional Certificate that must be renewed, and hold a salary classification of master's degree + 15 or less, or
You have completed coursework in a county-verified shortage area.
Note: There is a 15 semester hours reimbursement lifetime maximum.
Form 36 applications are accepted July 1 through June 15 of each fiscal year. They are processed on a first come, first served basis until the designated funds for that fiscal year are expended. All coursework must be completed within the same fiscal year in which we receive your request for reimbursement.
Applicant Requirements
General Requirements
To apply, you must:
Be a U.S. citizen or hold eligible documentation (see WVBE Policy 5202).
Be employed by a West Virginia public school system.
Hold a professional teaching, student support, or administrative certificate or first class permit.
Have a minimum 3.0 GPA is for coursework.
Be seeking an additional endorsement in a shortage area, if applying under Option Two.

Download the form and fill out the applicant information, disclosure of background information, and the details of the courses you are claiming for reimbursement.
Please note the required information. You must include:
A receipt verifying payment.
A transcript or grade report.
This signature of your county Superintendent verifying that your eligibility.
Ensure both you and your county Superintendent have signed and dated both pages of the form.
