Instructions

In addition to the online application, please be prepared to upload or mail in items such as:

• List of continuing education or training;
• List of community service activities, committee/organization memberships, awards, recommendations or recognition;
• List of exemplary performances by the Nominee;
• Publicity Headshot Photo. Guidelines;
  • 5″x7″
  • Minimum 1000px wide.
  • Color
  • Portrait Orientation
• Two letters of recommendation (one from the nominee’s principal/immediate supervisor AND one from administrator, colleague, teacher, parent, community leader, or student);
• Statement from Nominating Committee; Acknowledgement Form
