Use Form 9 to fulfill one of the following options:
Option One (1): Request an official, seal-bearing copy of your certificate issued by the Department
Option Two (2): Request to change your name or address as it appears on your certificate issued by the Department
To request a name or address change, you must provide verification of the change, such as a copy of a social security card, marriage certificate, drivers' license, or government-issued ID. Your updated certificate will only be available online as a PDF unless you also request an official copy and include the processing fee.
Submission
If you select to receive an official copy of your certificate, make your payment online before submitting Form 9. There is no fee for requesting a name or address change. You may email your Form 9 supplemental document to certification.wvde@k12.wv.us.
