Submit the Release of Information form to obtain a copy of documentation you have on file with our office. You:
Will need to include your license number or your social security number, the information you are requesting, and your contact information
Must include a copy of an official government-issued photo ID with the form
If the fee and a photo ID are not submitted, your request will be denied.

Make your payment online before submitting the form. Please see additional submission instructions on the form. You may email your Release of Information supplemental document to certification.wvde@k12.wv.us.
