First-Class/Full-Time Permit for School Nurse

Initial Application

• Validity period: One (1) year
• General requirements: U.S. citizen or hold eligible documentation (see Policy 5202); have recommendation from the county Superintendent; at least 18 years old
• Education level: A nursing diploma or associate’s degree in nursing and holds a valid registered nurse license issued by the West Virginia Board of Examiners (the applicant shall complete a bachelor of science in nursing from an accredited college or university within five (5) years)
• Grade point average: A minimum overall cumulative GPA of 2.5
• Coursework/training: Be enrolled in a state-approved program leading to licensure as a school nurse or a bachelor’s/master’s degree in nursing to satisfy the requirements to become a School Nurse at the conclusion of the five (5) year period
• Testing: No
• Employment required: Yes, the vacancy must have been posted with no certified individuals making application and an offer of employment must be made
• Job posting required:  Yes
• Official recommendation: Superintendent
• Official transcripts (when applicable): To send official transcripts to our office electronically, please them to cert.transcripts.wvde@k12.wv.us. If you select to send them electronically, they must be sent directly from the college or university to the Office of Certification.

Renewal

• Validity period: One (1) year (renewable up to 4 times for a total of five (5) years).
• Grade point average: A minimum 3.0 GPA for all required coursework
• Coursework/training: Six (6) hours of required coursework
  • School nurses are only required to submit three (3) hours towards meeting the requirements for a bachelor's degree in nursing during the first year, due to the lack of coursework availability. The signature of the designated institutional official, verifying the applicant’s enrollment in the state approved program, is required.
• Credential: Hold a valid or expired West Virginia First-Class/Full-Time Permit, renewing for continued employment in the current position
• Testing: No
• Employment required: Yes
• Official recommendation: Superintendent
• Official transcripts (when applicable): Unofficial transcripts or a grade printout reflecting completed coursework for renewal

Application Instructions

If this is your first time applying for a license, permit, or certificate with the Department, follow the instructions for first-time applicants.

1. Download the application form.
2. Fill in the applicant information. Work with your employer and your college or university to get the necessary signatures.
3. Pay the application fee online before submitting your application.
4. Submit your application and all supporting documentation to the Office of Certification. Please mail to the following address:
