Fill out this form in conjunction with a Form 20P application to convert an Alternative Teaching Certificate to a Provisional Alternative Teaching Certificate.
The Form 25C application verifies the completion of a county-based alternative certification program.
Before You Apply
To convert an Alternative Teaching Certificate to a Provisional Teaching Certificate, you must have:
A valid Alternative Teaching Certificate
Recommendations from your employing school district Superintendent, principal and an additional member of the professional support team
Completion of an approved West Virginia Board of Education county-based alternative certification program
Have passed West Virginia Board of Education-required Praxis exams
The Praxis Bridge/Alternate Pass Option is not available to completers of alternative certification programs that do not provide both content and pedagogy
A Form 20P application must be submitted in conjunction with the Form 25C application
Be physically, mentally, and emotionally qualified
Have recommendation from the county Superintendent
Hold a bachelor’s degree with a qualifying GPA from an accredited college or university
Have completed a state-approved alternative program or pathway
Have passed West Virginia Board of Education-required Praxis exams
