Use this form to apply for an authorization to teach a specific school program or subject area that isn't covered by a standard Professional or Career and Technical Education certificate.
Be employed in the West Virginia public school system
Be at least 18 years old
Be physically, mentally, emotionally qualified and of good character
Have recommendation from the county Superintendent
Requirements vary depending on the specific authorization you are seeking. Authorizations are outlined within the Form 30 application. For a full list of requirements, please review Policy 5202.
Please note: If your permit requires a Professional Certificate, that certificate must be active and up to date.
To send official transcripts to our office, please have your college or university email them to cert.transcripts.wvde@k12.wv.us. Transcripts must be received from the college or university, not the applicant.
