Use this form if you are in a position related to a public school instruction specialization or program not on the Professional or Career and Technical Education Certificate.
A Temporary or Permanent Authorization that requires a Professional Certificate as a prerequisite must be valid.
Other requirements differ depending on your credential. Speak with your employer or review Policy 5202 for more details. If you are employed in a county public school program, you must also have a recommendation from the county superintendent. If you are not employed in a county public school program, you must also submit Form 4B: Character Reference with your application.
To send official transcripts to our office, please have your college or university email them to cert.transcripts.wvde@k12.wv.us. Transcripts must be received from the college or university, not the applicant.
