Form 42, Permanent Authorization, is issued to applicants who hold a current Professional Teaching Certificate and need to modify an endorsement to match current grade level endorsements.
To apply for grade modification, the following requirements must be met:
Hold a valid West Virginia Professional Teaching Certificate in the endorsement are of requested grade modification
Meet teaching certification requirements in Policy 5202

Be a U.S. citizen or hold eligible documentation (see Policy 5202)
Be at least 18 years old
Be physically, mentally, and emotionally qualified
Hold a valid West Virginia Professional Teaching Certificate in the endorsement are of requested grade modification
