• Hold a valid (unencumbered/no deficiencies) Professional Teaching Certificate.
• 10 years of full-time teaching experience in a public school or eligible school.
• Held a valid Professional Teaching Certificate for each of those 10 years.
• Received satisfactory evaluations each year during the 10 year period.

*A verification of employment (Form 4PE, page 3 of the Form 4P application) for the required 10 years must be submitted with this form. If employed in different counties/districts, each employer must complete a form.

General Requirements

To apply, you must:

• Be a U.S. citizen or hold eligible documentation (see Policy 5202)
• Have recommendation from the county Superintendent
• Be at least 18 years old
• Have documentation from your county Superintendent and designated official at your college or university through which you are completing your program confirming that required coursework was not available during the school year

Application Instructions

1. Download the application form.
2. Fill in the applicant information. Work with your employer to get the necessary signatures.
3. Pay the application fee online before submitting your application.
4. Submit your application and all supporting documentation to the Certification Official at the county school system to be uploaded to the Office of Certification.
