Permanent Teaching Certificate HB3125

Prior to applying, you must:

• Hold a valid West Virginia Professional Teaching Certificate (certificate 21 or 22)
• Have 10 years of full-time teaching experience in a public school or eligible school
  • You must have held a valid Professional Teaching Certificate for each of those 10 years.
  • You must have received satisfactory evaluations each year during the 10 year period.

A verification of employment (Form 4PE, page 3 of the Form 4P application) for the required 10 years must be submitted with this form. If employed in different counties/districts, each employer must complete a form.

General Requirements

To apply, you must:

• Be a U.S. citizen or hold eligible documentation (see Policy 5202, Minimum Requirements for the Licensure of Professional/Paraprofessional Personnel and Advanced Salary Classifications)
• Be at least 18 years old
• Have recommendation from the employing county Superintendent or submit a Character Reference Form 4B if you have not been employed within the last twelve months

Please note: The designated county official where the experience and evaluations were completed must complete the Form 4E, sign it, and notarize it.
