The Release of Background Check Information (Form 7) is required as part of the licensure or employment process when a background check is necessary. Completing this form allows us to receive and use criminal history and background check information for official certification and licensure purposes.

Form MR is used to determine eligibility for certification through the Qualifying Veterans pathway, established by state legislation and implemented in accordance with WVBE Policy 5202, HB 3125 (2025), and SB 765 (2025).
