To ensure a positive and supportive learning experience, county school districts and schools should engage in a data-driven planning process, identify needs, and implement interventions targeting those needs. To support that process, School Learning Environment Surveys are provided so schools can gather information about the learning and teaching environment as well as the social-emotional well-being of students.
As part of the data-driven planning process, schools may conduct three interrelated surveys:
Student;
School Staff; and
Parent/ Caregiver
These surveys are conducted online and provide critical information about the learning and teaching environment, the health and well-being of students, and support for parents, school staff, and students.
The WVDE offers schools an opportunity to participate in the surveys once per school year, during the spring semester. Notification of the survey window dates are provided in the weekly Superintendents Update. The WVDE makes every effort to provide participating schools with reports of their survey results in as short a time as possible following the close of the survey window.

Parental Consent Forms
Parental informed consent is an important and necessary step for the student surveys. Generic parental consent forms are contained in the appendix of the guidance document, however, links to both pdf and MS Word format versions also are provided below.
