Certification Applications

Advanced salary/advanced degree recognition is limited to those who hold a Professional Teaching, Student Support, or Administrative Certification or equivalent.

Form 4A allows you to renew your provisional administrative certificate for five (5) years. It also allows you to renew your professional administrative certificate for five (5) years or covert to a permanent certificate.

Renew your Professional Student Support Certificate for three (3) years, or convert to a five-year or permanent certificate.

Form 4T allows you to renew your Professional Teaching Certificate, or convert a certificate to a five-year or permanent certificate.

Add an endorsement to your Professional Teaching, Administrative, or Student Support Certificate based on the completion of coursework at a regionally accredited college or university.

Add an autism endorsement to an existing WV Professional Teaching Certificate with a Special Education endorsement.

Use Form 8C if you want to add an endorsement to a valid West Virginia Professional Teaching Certificate after successfully completing a West Virginia Board of Education-required content assessment.

Use Form 8L to add new endorsements to a valid Long-Term Substitute Permit.

Add an endorsement to your Teaching, Student Support, or Administrative Certificate that is reflected on your renewable, out of state certificate that is not reflected on your current and valid WV certificate.

Add a Superintendent endorsement to your West Virginia Administrative Certificate following completion of the WVDE New and Aspiring Superintendent Leadership Training.
