Certification Applications

This permit may be granted only one time to an individual who has been continuously employed for a specific assignment under a First-Class/Full-Time Permit and who has completed all course requirements of the approved program but has not attained the required passing scores on the Praxis content exams.

This form is used to apply for a West Virginia Teaching Certificate if you attended a regionally accredited out-of-state college or university's state approved teacher preparation program.

Use this form to apply for a teaching certificate if you have completed an approved West Virginia Board of Education program as part of the Option 3 pathway (non-college/university).

Convert a Provisional Teaching Certificate to a Professional Teaching Certificate.

The Provisional Teaching Certificate is issued to individuals who complete an alternative certification program or hold a West Virginia Statement of Eligibility.

Renew a Provisional Teaching Certificate if you have not met the requirements for a Professional Teaching Certificate.

Use Form 20R if you are applying for a West Virginia Teaching Certificate and have a valid out-of-state Professional Teaching Certificate.

This form allows individuals who have attended an in-state college or university's state-approved teacher preparation program to apply for their Professional Teaching Certificate.

Teaching certificate for individuals who have completed Option 4: West Virginia Department of Education Alternative Certification Program.

Request for an observation or clinical experience in the West Virginia public school system from an out-of-state college or university.
