Certification Applications

Use this form if you are employed, but do not meet the requirements for a professional certificate, and are enrolled in a state-approved educator preparation program.

If you attended an in-state institution, this certificate allows you to work in the West Virginia public school system as a Principal, Supervisor, or Superintendent.

If you attended an out-of-state institution, this certificate allows you to work in the West Virginia public school system as a Principal, Supervisor, or Superintendent.

Convert a Provisional Administrative Certificate to a Professional Administrative Certificate.

If you have a valid and renewable out-of-state professional administrative license, this form allows you to work in the West Virginia public school system as a Principal, Supervisor, or Superintendent.

This permit may be granted only one time to an individual who has been continuously employed for a specific assignment under a First-Class/Full-Time Permit and who has completed all course requirements of the approved program but has not attained the required passing scores on the Praxis content exams.

Form 4A allows you to renew your provisional administrative certificate for five (5) years. It also allows you to renew your professional administrative certificate for five (5) years or covert to a permanent certificate.

Add an endorsement to your Professional Teaching, Administrative, or Student Support Certificate based on the completion of coursework at a regionally accredited college/university.

Use this form to verify your wage earning/work experience.
