Certification Applications

Use this form if you are employed on a first class/fill-time permit for a county but you do not meet the minimum grade point average requirement.

This permit may be issued to an individual who does not meet the requirements for the Professional Student Certificate issued for school counseling, speech-language pathologist, school psychologist, or attendance director but is enrolled in a state-approved educator preparation program and has been determined by the county superintendent to be the most qualified applicant for the position.

This permit may be issued to an individual who does not meet the requirements for the Professional Student Certificate issued for School nurse but is enrolled in a state-approved educator preparation program and has been determined by the county superintendent to be the most qualified applicant for the position.

This permit may be granted only one time to an individual who has been continuously employed for a specific assignment under a First-Class/Full-Time Permit and who has completed all course requirements of the approved program but has not attained the required passing scores on the Praxis content exams.

Authorizations are issued to individuals working in a public school-related specialization not included in the Professional or Career and Technical Education Certificate.
