Use this form to request a change in County or a change in Multi-County Center to the Office of Certification.

Use this form if you have been offered full-time employment by a West Virginia public school system but do not hold a professional certificate in the endorsement area, nor do you meet the minimum grade point average requirements.

This permit may be issued to an individual who does not meet the requirements for the Professional Student Certificate issued for school counseling, speech-language pathologist, school psychologist, or attendance director.

This permit may be issued to an individual who does not meet the requirements for the Professional Student Certificate issued for School nurse but is enrolled in a state-approved educator preparation program and has been offered full-time employment as a school nurse.

The Restricted Pre-Professional Permit may be granted only one time to an individual who has been continuously employed for a specific assignment under a First-Class/Full-Time Permit and who has completed all course requirements of the approved program but has not completed the required Praxis content exams.
