Certification Applications

Use this form to request a change in County or a change in Multi-County Center to the Office of Certification.

Use this form if you are employed, but do not meet the requirements for a professional certificate, and are enrolled in a state-approved educator preparation program.

This permit allows someone who usually works full-time as a college or university instructor to teach K-12 students in a specific subject area, as long as they’re already part-time employees in K-12 and have been recommended by the county superintendent.

Advanced salary/advanced degree recognition is limited to those who hold a Professional Teaching, Student Support, or Administrative Certification or equivalents.

Use this form if you are employed in the West Virginia public school system, but do not meet the requirements for a professional certificate and are enrolled in a state-approved alternative educator preparation program.

Use this form in place of Form 1 for Autism if you hold a Special Education endorsement and are taking six (6) hours of coursework at a college or university that does not offer a full Autism endorsement program.

Use this form if you are employed on a first class/fill-time permit for a county but you do not meet the minimum grade point average requirement.

This permit may be granted only one time to an individual who has been continuously employed for a specific assignment under a First-Class/Full-Time Permit and who has completed all course requirements of the approved program but has not attained the required passing scores on the Praxis content exams.

This form is used to apply for a West Virginia Teaching Certificate if you attended a regionally accredited out-of-state college or university's state approved teacher preparation program.

Use this form to apply for a teaching certificate if you have completed an approved West Virginia Board of Education program as part of the Option 3 pathway (non-college/university).

Convert a Provisional Teaching Certificate to a Professional Teaching Certificate.

The Provisional Teaching Certificate is issued to individuals who complete an alternative certification program or hold a West Virginia Statement of Eligibility.
