Certification Applications

This certificate allows paraprofessionals to work in a classroom under the supervision of a professional educator.

Form 4A allows you to renew your provisional administrative certificate for five (5) years. It also allows you to renew your professional administrative certificate for five (5) years or covert to a permanent certificate.

Form 4P allows you to convert your Professional Teaching Certificate to a permanent certificate if you have a valid, West Virginia professional

Renew your Professional Student Support Certificate for three (3) years, or convert to a five-year or permanent certificate.

Form 4T allows you to renew your Professional Teaching Certificate, or convert a certificate to a five-year or permanent certificate.

Fill out Form 55 if you are employed for a specific assignment in a county as a Professional Accountant, a Professional Business Official, or a School

The School Nurse Certificate may be issued to an individual who has been employed as a substitute or full-time School Nurse in a WV public school

Fill out this form if you have been hired by a county as an Educational Sign Language Interpreter.

Use this form to convert a Non-United States Citizen Permit to an Initial Professional Certificate for Teaching, Administration, or Student Support

Request a copy of your West Virginia Teaching Certificate or change the name and/or address for that certificate.

Receive a salary increase for additional coursework you have taken in addition to your degree.

You are eligible for this license if you teach Adult (basic) Education coursework.
