Certification Applications

Use Form 36 to request state tuition reimbursement.

This fee reimbursement is only for teachers who are employed in the West Virginia public school system and hold, or are working to acquire, National Board for Professional Teaching Standards (NBPTS) Certification.

Authorizations are issued to individuals working in a public school-related specialization not included in the Professional or Career and Technical Education Certificate.

Form 4NT can be used to renew a permit issued to a non-US Citizen to work as a teacher in WV public school classrooms.

Form 4T allows you to renew your Professional Teaching Certificate, or convert a certificate to a five-year or permanent certificate.

Authorization to instruct students in the operation of motor vehicles.

This authorization is issued to an individual who is employed as the lead teacher in a collaborative Pre-K setting.
