West Virginia Department of Education Grievance Procedure: Title II/Section 504
The grievance procedure may be used by anyone who wishes to file a complaint alleging a failure to accommodate and/or discrimination on the basis of disability in the provision of website services, activities, programs or benefits by the West Virginia Department of Education.
The complaint should be in writing unless an alternative method for filing is requested, such as a personal interview or an mp3 recording of the complaint. It should contain the following information about the alleged discrimination: 1. Your name, address, phone number and email address; 2. The name of the website against whom the complaint is filed; and 3. A statement of the problem.
Your complaint should be submitted by you or your designee as soon as possible, but no later than 60 calendar days after the alleged violation to:

As may be appropriate in a given case, and within 48 hours after receipt of the complaint, the Title II/Section 504 Coordinator will begin an investigation. As may be necessary, the Title II/Section 504 Coordinator or an assigned liaison will contact you to discuss the complaint and possible resolutions. As a general process, within 45 calendar days from receipt of the complaint, the Title II/Section 504 Coordinator or assigned liaison will respond to you in your preferred format (electronic format (ASCII, mp3 file, etc.), standard print, large print, etc.).
If the response by the Title II/Section 504 Coordinator or liaison does not satisfactorily resolve the issue, you or your designee may appeal the decision within 15 calendar days after receiving it to:
