Schools must have a process to ensure every student updates the PEP annually. Each PEP must have a signature page documenting the involvement of the counselor and/or advisor, student, and parent/guardian. This is a sample signature page if a paper format is being used. Additionally, this form will document student data (inventories, assessments, career exploration activities, etc.) used to guide PEP development.
